Compassionate and Personalized Care
We work with each client to help them and their families choose the level of care that best fits their needs. We offer a variety of services, including:
Personal Care
- Assistance with moving in/out of bed, wheelchairs, and motor vehicles
- Bathing and personal hygiene
- Using the toilet
- Dressing and grooming
- Eating, including meal prep and clean up
Homemaking
- Medication reminders or cues to take medications
- Preparing meals/assisting in the kitchen
- Changing bedding
- Vacuuming of client’s bedroom, bathroom, and common areas.
- Help with laundry
- Wet and dry mopping of the bedroom, bathroom, kitchen, and common areas.
Companionship

Our services are free for those on full Medicaid and who are eligible for the Aged & Disabled Waiver
Care Coordination
In addition to home care services, we partner with community organizations to provide care coordination services, including helping our clients:
- Apply for Medicaid
- Find housing
- With moving assistance
- Connect to community centers
- Find counseling
- Connect to resources and services that are available to them
- Locate community events that promote health, wellness, and safety
Here’s How it Works:
1. Get Assessed
A case worker will visit you at home to determine if you’re eligible for Medicaid-funded in-home care. You’ll find out how many weekly hours of care you can get.
2. Choose a Caregiver
You choose a caregiver: your daughter, son, relative, or friend. We come to your home to enroll you in the program.
3. You Get Care – They Get Paid
You get ongoing care from a person who loves you. They get peace of mind because they’re getting paid for the care they give.
Who is eligible for Dynamic Home Care?
Funded by Medicaid, we want to make sure understanding your eligibility* is simple.
You are likely eligible if you…
- You currently have Medicaid
- Live in Indiana
- Need help with day-to-day activities
- Able to direct your own care/services (if not, have an individual representative to appoint)
- Over 65 or have a recognized disability, and meet the requirements of the Aged and Disabled Waiver
- (Due to government regulations, Medicare is NOT enough.)
